In order to sell and redeem Book Sense Gift Cards, stores are required to sign and return the Book Sense Gift Card Agreement and Order Form (PDF)

Use the above link to download a PDF containing three documents (1) Book Sense Gift Card Program Participation Agreement, (2) Gift Card ACH Agreement, and (3) Gift Card Program Card Order Form.

Note that the Participation and the ACH Agreement contain important information for your review. Both documents require an authorized signature. Please fax all pages to ABA at 1-914-591-2724.
